did not expect the wheel to falloff. that was pretty calm driving
 
What a surprise that was, eh?

What make are those wheels? The hex molded into the wheel is unsupported by the webbing. And it looks thin wall there too. Possibly it is flexing and opening up during rotation allowing the hex to spin inside it? Check the hex fit into the wheel by pressing a hex into it with the wheel off the car. Should be a snug fit. Should not be a loose fit. Check with both the aluminum hex and a nylon hex. See which mates better.

What make are the aluminum hexes? Looks like they rounded off on one or two edges. If they are 6061 or better grade aluminum hexes, they should not be doing that either.
