I have seen it happen a few times.
Every time was due to the cooling head hitting the ground and causing damage to the block.
If the cooling head shows no sign of damage it was most likely replaced.
This can also happen if too long of heads bolts are used. The block is only tapped through 3 fins and if the bolts bottom out on the block pressure is applied to the upper half.
