Even if your slipper is new, if it slipped excessively, even once, it could be glazed. This would make it possible to slip, even fully tightened. That's why we always tell folks to clean up slipper pads and clutch shoes and bells with fine grit sand paper when they have that stuff apart. Then blast them off with nitro cleaner or da. Even if it isn't the problem they're working on. A couple drops of nitro fuel on the clutches wreaks havoc as well.
