Looks like its slipped around the pin a few times. Will this hurt anything? If you have any PM me.
 
Sounds like you didn't quite get the hex in right. Invest in some clampable hex adapters and get some new rollers. If you let it go I can imagine the one axle the wheel is attached to will over spin. Over heat? maybe. Over working your diff? maybe. fix it soon.
 
Only one is bad off but I wouldnt mind having 2 or 3 just for spare.
