Your spur gear nut is too loose and its slipping when you open the throttle up. tighten that down then back it 1/8 of a turn. Your slipper pad might be shot aswell. That's all I can say until you tighten that because that is most likely why it's not shifting

When its at full throttle does the spur gear spin like its at full throttle-very fast -or does it spin about as fast as the truck is moving-very slow?If the gear is spinning fast most likely as Gnarkill said but if it is spinning very slow it will be a clutch or clutch bell problem

Oops, I forgot to write the other half of my post. Silverbird got my back though.

Usually, it's one of these three or a combination of them:

1) Your idle screw is set too low. You should use the carb to adjust the idle, not the radio trim. When you apply brakes with an improperly set idle screw, it closes the carb and kills it.

2) Your clutch shoes/springs are damaged and are causing the clutch to drag at idle speeds which causes the stall.

3) Your tune is too rich on your LSN or really rich on your HSN causing it to load up with fuel and choke itself out when the RPM's drop.
