Depends on the body. If it's riding high enough, you can sometimes pull the cord out through the rear wheel well to start it. Or you could just cut a hole in the bed big enough to pull the PS cord up through.
 
Well, you don't want it flopping around... you can put a bit of fuel line between the handle and the case to make it stick up more. Then you can keep tension on it while it's running.

This is a short amount of fuel line, but it gives you the general idea:
